As a Federally Qualified Health Center (FQHC), CHN has earned the distinguished Patient Centered Medical Home (PCMH) Level 3 seal, the highest level possible, proving we go the extra mile to ensure patients receive outstanding care. CHN runs several integrated medical and behavioral care clinics in the area, making it one of the few local organizations that provide this type of integrated care.

The Center for Respite Care provides quality, holistic medical care to people experiencing homelessness who need a safe place to heal, while assisting them in breaking the cycle of homelessness.

The Center for Respite Care (the Center) is a 24-hour facility providing medical and nursing care to sick people experiencing homelessness to promote effective healing. Clients receive quality medical care and assistance in breaking the cycle of homelessness.

Since opening in 2003, the Center has cared for over 1,700 sick and injured people who are homeless.

While it’s true that dependency and addiction often go hand-in-hand, proper assessment and diagnostic impressions are crucial to determine the severity of one’s condition.  CAT uses a multi-disciplinary team to assess and create an individualized treatment plan.
